A claim statement is a written document that is prepared by the consumer (for example, the buyer) and sent to the manufacturer of the goods to which there are complaints, or to the head of the outlet where the defective goods were purchased. Despite the fact that the claim statement is considered a document of any sample, it is worth adhering to certain rules in its filling.

Write in the header of the letter to whom you are sending the claim. It is advisable to indicate the name and position of the head, as well as the name of the organization and its legal address. If this data is not known to you, just write the word "Claim", without an introductory part.

In a business style, without the use of discounted vocabulary and preferably without unnecessary emotions, describe the situation. Be sure to state the facts. For example: "On such and such a date in your store I purchased such and such a product at such and such a price." Describe what exactly the product did not suit you.

After describing the problem with the product, refer to the Consumer Protection Act. As a rule, a claim statement indicates article 29 of this law, which refers to the consumer’s right to terminate the contract of sale or provision of services.

Complete the application with your requirements, for example: "I ask you to return the money paid to me under the sales contract in the amount of such and such an amount." If you intend to assert your rights in court, write about it so that the potential defendant is warned.

Attach all relevant documents to your claim statement. For example, warranty card, cashier's check, sales contract. Use copies, keep originals.

At the end of the letter, indicate your details: name, contact phone. Sign the claim statement with a ballpoint pen and make two copies. On one copy, ask to sign the person who will accept your claim. Keep the signed statement until further proceedings.
