When applying to the FMS of Russia at the place of residence you will be given an application form for issuing (replacing) a passport. It must be filled in legible handwriting. In the first line, write the last name, first name and patronymic, for example: Elena Petrova.

Write your date of birth: day, month, and year. For example: April 30, 1984.

Indicate the place of your birth. This can be a village, town, city, district, region, region. For example: Moscow.

You must specify your gender. For example: Female.

Indicate marital status. If you are not married: single / single. If you are married, you must specify the last name, first name and patronymic of the wife (husband), which registry office registered the marriage. For example: Petrov Oleg Andreevich, Nagatinsky registry office in the city of MOSCOW.

Surname, name, patronymic of father and mother. Indicate the initials of your parents: Krasnov Ivan Anatolyevich, Krasnova Tatyana Sergeevna.

Place of residence, stay. Cross out the unnecessary. In this line, indicate the name of the region, district, your settlement, street name, house number, building and apartment. For example: Moscow, st. Nagatinskaya embankment, 62 24.

The eighth line must indicate whether you were a member of a foreign state before and when you accepted Russian citizenship. This applies to those who were previously a citizen of another country. If this does not apply to you, write: did not belong. If you used to live in another country, indicate its name and date of adoption of Russian citizenship. For example: United States of America, February 18, 2001.

In the next line, indicate in connection with which you need to issue (replace) a passport. The reason may be a change of surname, reaching a certain age, loss or theft of a document. For example: I ask you to issue (replace) a passport in connection with a change of surname. Below indicate the date of filling out the application and your signature.

In the next line write the details of your old passport: series, number, when and by whom it was issued.

All citizens of Russia must change their passport when they are 20 and 45 years old.
