A certificate or certificate of incapacity for work is issued by a medical institution. It confirms the temporary or complete disability of an employee, student or preschooler. Prepare an insurance policy, passport and additional documents that may be needed at the hospital. After that, call a doctor at home or go to a medical facility.

Take help form 095 / y. Indicate in the leaflet the address and name of the healthcare institution. This is necessary so that inspection authorities can verify the authenticity of your disability certificate. Fill in the column indicating the "primary" or "secondary" case of disability, and also indicate the date of issue of the certificate. In this case, the date and year are indicated in numbers, and the month in words. Surname, name, patronymic of the disabled person fill in completely, without abbreviations. Indicate the full age of the patient in numbers, without date of birth. Indicate the place of work or study and the position occupied by the disabled.

Fill in the column for the diagnosis of the patient. From this moment, the certificate should be filled out by a doctor or under his supervision. In some cases, the patient’s diagnosis may not be indicated due to the confidentiality of the information. Indicate the reasons for temporary or complete disability. For inpatient care, note the date of hospitalization and discharge. If you were referred to MSEC, then indicate the date of sending and the conclusion delivered by MSEC.

Determine the period of temporary incapacity for work and indicate the estimated date of going to work or study. If the doctor diagnoses complete disability, then put the appropriate seal. If temporary incapacity for work is extended, then the doctor should say "continues to hurt." The doctor indicates his position and surname and certifies the certificate with his signature.

Assure the received certificate of incapacity for work at the head physician. The certificate must bear the round seal of the medical institution. Register the issuance of the certificate in the journal of control of the issuance of certificates of disability.
