If it is necessary for a citizen to receive important information that is at the disposal of a state body, an official request is drawn up. Any request, regardless of the subject of the letter, should have a certain form.

In the upper right corner, indicate the last name, first name, middle name and full name of the position of the person to whom you are sending a letter, for example: Ivan Ivanov, the Commander of the Caspian Flotilla of the Ministry of Defense of the Russian Federation. Having stepped back one line, in the same column write your last name, first name, middle name with the address of your permanent location, email address and phone. This is necessary so that a response to an official request is sent to your name by mail, and in case of any problems or the need for clarification, contacted by phone.

Next, approximately in the middle of the sheet, write the name of your document. This can be an “Official Request”, “Request for Information” or “Information Request”. Begin your request by contacting the recipient of the letter, for example, "Dear Ivan Ivanovich!" Next, from the red line, begin to state the essence of your appeal. Rely on legislation and regulations on the immediate subject of the letter. After all, the more fully you know your rights under the law, the more likely it is that your letter will be treated more responsibly.

After describing your problem, back off a few lines and, in capital letters, the word "please", indicate what information you are interested in. Write clearly, concisely and informatively, enter the position of a person who receives dozens of official requests, if not hundreds, then every day. Formulate your request so that there is no doubt that you are asking. Any ambiguity can affect the fact that you will be answered with common phrases, and you will have to send the request again.

At the end of the letter, be sure to sign and date.

The representative of the state body to whose address you sent the letter is obliged to respond to the official appeal within a month from the day the letter was received by the organization.

On most websites of state bodies, there are forms of online appeals that will be sent to the official’s address much faster than a regular letter.
