Accept from the employee an application for amending personal data in connection with a change of surname (for family and other reasons). Issue an order on the need to make changes to all documents of the employee, which are under the responsibility of the personnel service and accounting. The order must be registered in the Journal of registration of orders. Familiarize the personnel department, bookkeeping and the employee himself under the order with the order.

Before making changes to the employment contract, a voluntary agreement must be concluded with the employee. After the conclusion of the agreement, the agreement can be amended by crossing out the previous name and inscribing a new one on the corresponding column. In confirmation of the legitimacy of the changes, indicate the name and number of the document on the basis of which the amendments were made (for example, certificates of marriage / divorce). After that, the data changed in the "Last Name" column are certified by the employer and employee.

On the basis of documents confirming the legitimacy of changes in the personal data of the employee, amendments are also made to the work book (on the first page or cover page). The previous surname is crossed out with one line, after which a new one is indicated on top of this entry. References to relevant certificates (and other documents) are indicated on the inside of the cover of the work book. And it is this record that is certified by the signature of the employer and the seal of the personnel department.

In the employee’s personal card, his previous name is crossed out with one line and a new one is indicated next to (or above). The name and number of the document on the basis of which changes are made are indicated opposite the column "Last Name". Information about the document can also be entered in the X column of the personal card ("Additional Information"). These amendments must be certified by a personnel officer.
