In the Orthodox Church, a believer has the opportunity not only to pray for someone on his own, but also to make sure that the name of a loved one is mentioned during a prayer service. To do this, you need to write a special note, which must be properly framed.

Go to church and find a minister who takes notes with a request for a moleben. Usually you can buy candles from him. In some cases, a box may be present in the church in which you must lower the note yourself. There will be an explanatory inscription on it. If you cannot figure out where to write the notes, contact one of the parishioners who are not currently praying, or a free clergyman.

Take the form for writing text. The word “ Prayer ” will be written above, and below you will have to write your prayer request.

Indicate why you should mention a person close to you in a prayer service. Most often they pray "For health" for the living and "For repose" for the dead. However, the wording may be more specific, for example, “On successful childbirth” for a pregnant woman or “On maintaining faith” for those who have religious doubts.

If you want the prayer to be offered to the Theotokos or any particular saint, you should write down his name. Such an opportunity exists during the Ascension Day.

Write down the names of those whom you ask to mention in this prayer below. There can be several names, for example, in the card “About Health” you can indicate all your currently living family members and friends. For a more personal theme, the prayer may be one name. In this case, it is required to indicate only proper names adopted at baptism. Surnames and patronymics of people are not needed.

Hand the form over to the minister of the church collecting them. Pay the required donation amount. It may depend on the number of notes submitted or the number of names mentioned. If you wish, you can specify the time of reading the prayer in order to attend it yourself.
