Make an appointment and find out all the necessary information about collecting documents to restore the loss (certificate of residence, certificate of BTI, forms No. 1, No. 9, No. 16).

After collecting all the documents, go to the appropriate institution for the recovery procedure. Pay a fine to Sberbank for the loss of a document and the state fee for issuing a new document. Write a statement addressed to the head of the institution. In the application, indicate the full name and the reason you ask for help. Put the date and list. Save the coupon that guarantees you the replacement of the lost document. Wait for 15 calendar days for a new document to replace the missing one.
