Take a piece of A4 paper. Some organizations have their own complaint forms - you can use them. The complaint must be made in duplicate. One is given to the institution you are writing the complaint about, and the second is yours.

Indicate in the upper right corner the name of the organization to which the application is sent. The position, surname, name and patronymic of the head of the organization are noted below in the dative case. Even lower in the genitive case is your last name, first name and middle name, address and telephone number. This is necessary for feedback from you, since anonymous complaints are usually not considered. On the left side of the sheet you need to indicate a summary of the complaint. For example, if you complain about the work of a housing department employee, then in this line write: "On illegal actions of a housing office employee". In the center of the sheet, write the word "Complaint."

Write a complaint text. In the description you must specify the maximum of the information known to you. Indicate the full name, position, rank, place of work, badge number, service certificate number, address and other data of the person to whom the complaint is written. Describe the circumstances in which your rights were violated, indicate the persons who can confirm the violation. If possible, indicate the exact name of the violation of consumer rights, rules or laws. If you have documentary evidence, attach copies certified by a notary to the complaint. At the end of the complaint, be sure to indicate the list of attached documents, below - put the date of writing on the left, and on the right - a signature with a transcript.

Give the complaint to the reception organization. Ask the complaint and your copy to put the incoming stamp, make sure that the dates on both copies are the same. The complaint is usually considered within a few days, and an answer can be expected within a month.
