In the article, we will consider the procedure for changing the last name and first name in accordance with the law, with the consent of both parents. Any child who has reached the age of 14 years has the right to change the name, patronymic or last name, if the child is less than age, both parents, guardians or a court order agree to change the name. The Family Code obliges parents, upon changing the name or patronymic of a minor child, to provide without fail the decision of the guardianship and guardianship authorities.

Then they fill out an application in writing and submit it to the registry office, it contains information about the place of residence and birth, full details of the patronymic, first name and surname, data on marital status, as well as all data on minor children. Originals and copies of all documents issued earlier by civil registration authorities are provided.

Signed a statement on the change of last name or patronymic, affixed the date of compilation. All collected birth certificates of children, registration of marriage or its divorce, are applied to the application.

Within a month from the date of filing the application, it is considered by the registry office, in case of refusal to change the middle name or last name, provide a reasoned reason for the refusal, and return all attached documents. Upon a positive review of the application, they inform about changes in the name change to the internal affairs bodies at the applicant’s place of residence.

Then they issue a certificate with the amendments and an indication of the place of the change of surname and patronymic. Further, the data is changed in all documents requiring updates.

In cases where the child is adopted, the change of name, patronymic and last name occurs in accordance with Article 134, which provides for the same registration procedure as the above. The exception is such a moment as the child’s mandatory consent to change his name, surname and patronymic if the child is 10 years old.
