If you have lost your passport, the only way to restore it is to contact the passport office of the Housing Office or the Federal Migration Service to obtain a new one with a set of necessary documents.
You will need
- - a statement about the circumstances of the loss of a passport;
- - a statement of the established form on the issuance of a new passport;
- - 4 photos 35 by 35 mm, color or black and white, full face on a light background;
- - receipt of payment of state duty.

If you have lost your passport, you are required to bring to the passport office of the Housing Office or territorial division of the FMS at the place of permanent or temporary registration or actual residence a written statement outlining the circumstances of the loss of the passport (usually written in the name of the head of the unit.

Fill in the application form for the issuance of a new document in form 1P. It can be taken at the passport office or the FMS unit, downloaded on the website of the regional FMS or FMS of the Russian Federation, or filled out online on the public services portal.
It will also require four photos of 35 by 45 mm and a receipt for the payment of state duty. Its size in 2011 is 500 rubles. You can download the receipt with the details on the website of the regional FMS.
If the documents are in order, you must issue a new passport within two months after their adoption.

In some cases, there may be problems identifying you. For example, for some reason, the file cabinet with your previous applications for issuing or replacing a passport has not been preserved. In this case, you will have to provide, upon request, any documents that will help establish your identity: birth, marriage or divorce certificate, military, trade union or hunting ticket, passport, certificate of release from places of deprivation of liberty, work book, driver's license, official, retirement or any other available certificate.
