Executive authorities, as a rule, are vested with control functions in relation to their subordinate institutions. Therefore, complaints about shortcomings in the work of such institutions and their officials - perhaps most often this applies to educational institutions and medical institutions - must be sent to the appropriate department. To get started, in the telephone help desk, find out the phone number of the department you intend to file a complaint with.

Then, by calling directly to the department, specify the following questions:
- whether the institution or officer you are going to file a complaint with is subordinate to this department;
- full name of the department;
- last name, first name, patronymic of the head of the department;
- department address and office hours.

Armed with the above information, you can start compiling a complaint.

There is no mandatory form for complaints; a complaint is drawn up in any form. Even the word "complaint" can be replaced, for example, with the words "statement", "appeal" or dispense with a heading altogether, replacing it with an appeal to the head of "Dear (Name, Patronymic)!"

In order to clearly indicate the sender and recipient of the complaint, as well as clearly provide your contact information, in the upper left corner of the sheet write the so-called “cap”, which should look something like this: “To the head of the Health Department of the Ensk Region
Petrov P.P.ot Ivanov Ivan Ivanovich living at:
Ensk, st. Such and such, house number 1, apt. Number 2
contact tel.: 89101234567 ".

Below the heading in the middle of the sheet, write, as mentioned above, the word "complaint", "statement", "appeal" or "Dear Petr Petrovich!"

In the text of the complaint, briefly and clearly state the essence of your appeal. Be sure to include the exact dates, times, last names, first names, patronymics and positions of persons who violated your rights, as well as other information necessary to specify the violation committed against you.

At the end of the complaint, state the purpose for which the complaint was filed or your request. For example, “I ask you to check the fact of the violations I described and inform me of the results, ” “I ask you to take measures to stop violations of the rights of patients at Clinic 10, otherwise I will have to turn to the prosecutor’s office” and other options depending on the specific situation.

Complete your complaint with your signature and current date.

So, the complaint is made. Before you file a complaint with the department, be sure to make yourself a photocopy of it.

There are several ways to file a complaint:
- in person, at a reception with the head of the department;
- personally through the secretariat of the department;
- by registered mail with a notification and a description of the attachment. If you are submitting a complaint in person, the person accepting the complaint is obliged to put photocopies of the complaint that remains with you: your name and initials, position, date of receipt of the document. by mail as confirmation of filing a complaint with the department you will receive a notice of delivery. Attach it to your photocopy of the complaint and keep them together. You must receive a response to your complaint within a month from the day the complaint was received by the department.
